Measurement before intervention
Longevity starts with knowing where you actually stand. The eight focus areas exist to measure the variables with the strongest evidence for predicting healthy lifespan: cardiopulmonary fitness, metabolic health, cardiovascular risk, strength and mobility, hormone status, genetic risk, and early cancer detection. Each year produces a clear picture of your trajectory, not a stack of disconnected lab results.
Intervene where the evidence is
Most of what moves lifespan is unglamorous: raising VO2 Max, building and keeping muscle, managing blood pressure and ApoB, sleeping well, catching cancer early. These are the levers with decades of evidence behind them, and they are trainable, measurable, and repeatable. Your physician builds the plan around your results and adjusts it as the data comes in.
What we do not do
We do not sell supplement stacks, chase biohacking trends, or promise age reversal. When a new intervention accumulates real evidence, we evaluate it honestly. Until then, we would rather tell you the truth than sell you a protocol.

- Not as a routine part of membership. The Optimal Care program is built around interventions with strong evidence: VO2 Max training, metabolic optimization, advanced cardiovascular screening, sleep and stress management, hormone screening, cancer screening, strength and mobility work. When new interventions accumulate sufficient evidence, we evaluate them. We do not chase trends.
- For most patients in their 40s and 50s, the answer is cardiopulmonary fitness. VO2 max is one of the strongest single predictors of all-cause mortality, and it is among the most trainable. We measure it twice yearly and build a training program around the result. For some patients, sleep is the bigger lever. We figure out which lever matters most for you specifically.
- Optimal Care is our diagnostic program. Longevity medicine is the broader practice of using those diagnostics, plus a continuous primary care relationship, to extend healthy life. Optimal Care is the toolkit; longevity is the goal.
