The patient panel at Kosmos spans high schoolers through retirees, often within the same family. The practice is designed for that. A child enrolled at 16 keeps the same physician through college and into their first job. A parent who joins at 45 keeps the same physician through retirement.

Coming of age

Ages 15 to 29

A trusted physician for the formative years.

A young adult does not need much medical care. What they need is a relationship with a physician who knows them, can answer questions about anxiety, sleep, sports injuries, and contraception, and is reachable by text. Kosmos is a low-friction way to build that relationship before college and keep it intact through early career.

The Glue

Ages 30 to 64

The decade chronic conditions arrive, set up to live better and longer.

Forties and fifties are when chronic conditions typically emerge. The Kosmos approach is to find them early through proactive screening and to manage them seriously through ongoing relationships. The Optimal Care diagnostics program is designed for this stage.

The Storyteller

Ages 65 and older

Ensure your last decades are your best decades.

Prompt diagnosis and treatment of acute conditions, focused care for chronic conditions, and a unique emphasis on strength, mobility, and balance. The goal is not just years. The goal is healthy years.

How the family pricing works

Children of Premium Care or Optimal Care members aged 15 to 29 can enroll for $200 per month each ($2,400 annually). Children under 15 must remain with their pediatrician.

Persons outside the nuclear family are considered a separate family and require a separate membership.
