Refer a friend or family member, get one month
Existing members receive one month of their annual fee credited for each converted referral who remains active for twelve months. There is no cap.
Insurance
Does Kosmos work with my insurance?
Kosmos is a Direct Primary Care (DPC) practice. We do not bill insurance or Medicare for our services.
We recommend you keep your insurance for everything outside our office: specialist care, imaging, labs, hospitalizations. Your Kosmos physician coordinates those referrals and stays involved throughout.

Children of members aged 15 to 29 can join for $200 per month each. The membership covers all primary care services delivered in our office; insurance is not billed.

- Concierge medicine is a model of primary care in which you pay a membership fee directly to your physician rather than relying solely on insurance billing. In return, you receive same-day or next-day appointments, unhurried visits, 24/7 direct access to your doctor by phone or text, and a level of preventive and personalized care that traditional practices cannot offer within the constraints of a high-volume schedule.
- A typical primary care physician cares for 2,500 to 3,500 patients, and most concierge physicians care for 600 to 900. Kosmos physicians care for just 100 families, so they have time to know your history, your goals, and what actually matters to your health. Visits run as long as needed, not 15 minutes. You reach your physician directly, not a triage line. And for members who choose Optimal Care, Kosmos layers in advanced diagnostics that most practices do not offer, including whole-body MRI, VO2 Max testing, and multi-cancer early detection.
- The clearest way to know is a 30-minute intro call. Most members come to Kosmos because they want a primary care relationship that goes deeper than insurance allows: more time, direct access, and proactive care. If your current primary care feels rushed or transactional, or if you want to be more proactive about prevention and longevity, Kosmos is likely a good fit. The intro call is unhurried and there is no pressure to enroll.
